Position Description

Subject to change based on final version of contract and various addendums.

Duties/Responsibilities:

  1. Accepts reviews and expands on initial assessment for family.
  2. Conducts needs assessment while encouraging and utilizing family voice/choice and family driven principles.
  3. Participates in and supports safety plan.
  4. Coordinates and provides services designed to meet the individualized needs of children/youth and families.
  5. Prepares draft copy of court reports and testifies as needed, providing expert testimony regarding case plan progress, child/youth and family strengths and barriers as well as related decisions.
  6. Coordinates MH/SA related documentation and incorporates into family driven plan and overall service delivery.
  7. Assures all required documentation is completed and provided to DHHS-CFS within the designed time frames.
  8. Organizes and facilitates Family Team Meetings on a monthly basis (or more frequently as needed) focusing on Safety, Permanency and Well Being.
  9. Ensures that services are implemented in the event of in-home or out-of-home placement and adjusts as needed to ensure progress.
  10. Assists family in developing their permanency goals by encouraging families to select individualized strategies and interventions that meet their needs.
  11. Assists children/youth and families in identifying, securing and working with informal support systems when appropriate to assist in meeting their individualized needs and case plan goals.
  12. Maintains on-going contact with Families F.I.R.S.T Manger, CFS, and Family Partners regarding each case assigned as deemed appropriate or as requested.
  13. Selects/provides services that will address critical domains of the YLS/CM for youth involved with the Juvenile Justice system.
  14. Selects services in conjunction with children/youth and families support skill development of the youth/family
  15. Drafts updates of the YLS/CM.
  16. Visits with children/youth and family a minimum of monthly in accordance with Federal requirements.
  17. Provides information to CFSS if negative reaction occurs regarding critical incidents experienced within or by children/youth or families.
  18. Works collaboratively with other team members to control, monitor and manage safety plans with families.
  19. Conducts protective capacity assessments and other assessments to assist in the elimination of a safety threats.
  20. Oversees and assures routine and emergency medical care for child/ren.
  21. Provides Independent Living (Adolescent Permanency) and Transitional Living services to children through age 21.
  22. Conducts ICJ home studies, ICPC to assure supervision and support to children.
  23. Maintains documentation information on N*Focus.
  24. Takes applications for guardianship/ adoption, along with relinquishment request and completes subsidy request where appropriate.
  25. Ensures collaborates with CFSS to ensure permanency hearings are held within 12 months post removal of the children along with subsequent reasonable effort hearing.
  26. Provides aftercare services for a period of 12 months post discharge and reports children/youth and family status to DHHS-CFS as requested within required timelines.
  27. Makes recommendations for case closure.
  28. Performs other duties as may be assigned.

  4. Timeliness of reports and services.
  5. Accuracy of reporting.

 

 

Relationships

 

1.       Supervised by: Program Manager

2.       Supervises: Indirectly supervises individuals performing services.

3.       Internal: Works in harmony with other team members and Families F.I.R.S.T. managers to achieve maximum outcomes for families.

4.       External: Continual and on-going contact with CFSS, Juvenile Courts, and related institutions.

 

Job Specifications and Work Requirements

1.       Education

a.        Required: Minimum of Bachelor Degree in Human Services or related field.

b.       Desired: Masters Degree in Human Services, Counseling or related field.

 

2.       Experience

a.        Required: Must be 21 years of age and have a minimum of 3 years experience as a Family Partner or Team Leader, or 3 years experience working children and families.

b.       Desired: Prefer prior case management experience.

 

3.       Licenses, Certifications, and/or Registrations: Clear Criminal History Background and Adult/Child Abuse checks, as well as all program specific certifications, NCI, First Aid/CPR, Car Restraint, Defensive Driving, clearance from all background checks and drug screens (Trainings offered by Visinet, Inc.)

 

4.       Equipment/Tools/Work Aids: Must have fluid knowledge of computers to include N*Focus, Microsoft Word, Excel, etc.; professional phone skills to receive and relay pertinent client or case information.  

 

5.       Specialized Knowledge and Skills: Prior case management and/or field experience in DHHS helpful. Spanish, sign language, etc. beneficial.

 

6.       Working Conditions:

a.        Varied climates both indoors and outside in homes, neutral locations or related areas.

b.       Some travel to attend meetings and court held appointments.
