Job Description

FRP is responsible for back-up and intake and discharge of all referrals as directed by the Families F.I.R.S.T Manager contributing to fidelity and quality of services provided according to Visinet’s mission, vision and values.
1. Completes all initial treatment/service requests per Medicaid Managed Care/State contracts before assigning cases.
2. Conduct weekly contact with the Family Resource Parent(s) regarding the status of the youth/child placed in the home.
3. Identifies goals, objectives and skill/support interventions to be followed from the master service plan.
4. Provide/assure that weekly supervision is being provided to youth/child.
5. Contributes to efficient and effective Family Resource Parent operation of program performance.
6. Adhere to Person/Family Centered practice at all times.
7. Maintain fidelity with the chosen EBP/PP models.
8. Maintains the mission of Visinet to create the best outcomes for youth/child/families.
9. Quality of services performed and related documentation of each service component.
10. Effective communication between CFS specialists, Family Resource Parents, Service Coordinators and other Stakeholders.
11. Maintains face-to-face contact with the Resource Family Parent(s) a minimum of two (2) times per month for a minimum of one to one and a half (1-1 ½) hours for all children place in the Resource Family home.
12. Compliance with all contractual and corporate policies and procedures.
Essential Functions of the Position and Performance Standards
Duties/Responsibilities:
- Obtain and ensure that all collateral documentation is in the client(s) file prior to initiating services.
- Ascertains that all of the required assessments/referrals/authorizations/Medicaid eligibility checks are completed prior to initiating services and providing services on an ongoing basis.
- Updates trackers regularly and maintain current authorizations on all clients at all times
- Reviews client files regularly (a minimum of monthly) to ensure compliance.
- Develop and implement safety plans for all Resource Partner families and children, specific to the needs of the client.
- Completes any necessary referral forms for services to include, but not limited to: outpatient therapy, psychiatric evaluations and psychological assessment within the first (2) weeks of placement or as necessary.
- Reviews all Family Resource Parent documentation and assess for appropriateness.
- Provide payroll/billing information to Billing Manager by set deadlines.
- Complete master service plans for all children/youth on case load within designated time frame.
- Prepare written monthly service plan updates
- Complete all documentation within forty-eight (48) hours (of conducting a session) and provide to administrative assistant to be filed.
- Complete incident reports and submit to the Families F.I.R.S.T Manager within twenty (24) hors of the incident.
- Complete outcome measurement tools for every new client.
- Consistently review Continuous Quality Improvement (CQI) plan and make sure that all is being carried out within the program on a daily basis.
- Ensure that Health Insurance Portability and Accountability Act (HIPPAA) standards are maintained throughout the program.
- Provide the necessary outcome measures information to the Clinical Director and Chief Operating Officer and Resource and Training Director.
- Problem-solve crisis issues in an effective and efficient manner; staff all clinical issues with the Manager, COO and/or Clinical Director immediately upon report of a crisis situation.
- Communicate needs of clients to appropriate members of the team, DHHS CFS Specialist, staff, biological parents (after ensuring that the necessary releases of information are in place.)
- Ensure that random and consistent field observations are completed on staff at various levels of their involvement with clients.
- Assist with obtaining ongoing authorizations on all clients on case load.
- Work with the Family Resource Parent(s) on transition issues for when a youth/child is transitioning to a different Family Resource home, back to their biological home, or to any other type of placement.
- Establish a working relationship and rapport with client(s)/families.
- Assist in maintain the youth/child in is/her Family Resource home.
- Assist the Family Resource Parent(s) on learning and implementing appropriate strategies to assist youth/child toward the goals and objectives on their individual service plan.
- Provide support to the Family Resource Parent(s).
- Assist in setting up respite for the Family Resource Parents.
- Assist in moving children to new Family Resource Parent(s) if disruption occurs.
- Consult the Families F.I.R.S.T. Manager; COO and/or Clinical Director on all potential liability issues.
- Collaborate with other professionals such as therapists, teachers, and probation officers to see that the youth/child’s individual needs are being met.
- Provide information feedback, creative quality improvement strategies, and ways to increase client referrals on a consistent basis.
- Assist Family Resource Parent(s) in identifying appropriate goals and objectives from the service plan to focus on.
- Assist Family Resource Parents in completing additional documentation such as medication logs, clothing inventories and incident reports.
- Assist Family Resource Parent(s) in assuring the youth/child has medical, dental and eye appointments within 30 days of placement.
- Provide support to the Family Resource Parent(s) including support outside the regular contact time, crisis calls and attending meetings.
- Provide planning interventions (i.e. teaching, coaching, practicing, role-playing, affirmation, etc.) based on individualized treatment plan.
- Secures funding for special activities and additional clothing or personal needs.
- Maintains a strengths-based approach while working with client(s) and families to assist in the retention of dignity and the development of positive intrinsic qualities.
- Adhere to the emergency protocol found in the Employee Manual in the event that a client or individual associated with the client discloses abuse/neglect information.
- Maintain appropriate boundaries and confidentiality with client(s) and families.
Performance Standards:
- Family Resource Partners will collaborate with Recruitment and Training staff regarding compliance and licensing concerns.
- Effective collaboration with clients and Family Resource Parent(s) coordinating a consistent schedule between child(ren) and the Family Resource Parents.
- Effective rapport with client(s)/families and biological families.
- Effective outcomes in assistance with the client/family in achieving identified goals.
- Outcomes of program specific standards.
- Quality of services and monthly reports.
- Rapport with family and referral sources.
- Assurance of completion to required trainings.
- Timely attendance at scheduled sessions and meetings.

External: Contact with clients, youth/child, families and Family Resource Parent(s).
Job Specifications and Work Requirements
- Education
- Required: Bachelor’s Degree in social work or related field .
- Desired: Bachelor’s Degree in social work and a minimum of one (1) year of experience working with at-risk families.
- Experience
- Required: Minimum of one (1) year of experience working with at-risk families.
- Desired: Two (2) to three (3) years of experience working with at-risk families.
- Licenses, Certifications, and/or Registrations: Clear Criminal History Background and Adult/Child Abuse checks, as well as all program specific certifications, NCI, First Aid/CPR, Car Restraint, Defensive Driving, clearance from all background checks and drug screens (Trainings offered by Visinet, Inc.).
- Equipment/Tools/Work Aids:
- Must be knowledgeable in Microsoft Office programs including Word, Excel and Outlook or similar types of software (Google Apps, etc.)
- Specialized Knowledge and Skills:
- Spanish, Sign Language, etc. beneficial
- Working Conditions:
- Varied climates both indoors and outside in homes, neutral locations or related areas.
- Varied working hours – typically late afternoon and early evenings and at least one weekend day.
- Various other components such as pets, housekeeping skills, etc.
- Significant time spent driving between and/or to sessions.
